Mediplacements are looking to recruit a band 6 Speech and Language Therapist for a locum position with our client in London.

The pay for this position is £23-£28 per hour, and we are looking for someone who can start as soon as possible for an ongoing duration.

Job Description:
The team works with clients whose needs cannot be met by their local therapy teams, thus clients typically present with complex cognitive, physical, communication, social and behavioural presentations requiring a neuropsychologically-led approach. They cover a wide spectrum, with some highly dependent on others for most activities of daily living, and others having more subtle deficits requiring high level cognitive or vocational rehabilitation. Interventions accordingly focus as appropriate to the client on a wide range of concretely defined objectives which may include increasing autonomy and independence within the home, enhancing social participation and psychological well-being, and facilitating resumption of important social roles including education or employment. The team has developed a specialist Vocational Rehabilitation pathway and works closely with employers and third parties to support clients to successfully return to work. Individually-tailored programmes are formulation-driven, goal-focused and time-limited, requiring coordination of interdisciplinary working from the whole team.

JOB SUMMARY:
* To work alongside your SLT colleague to provide the specialist Speech and Language Therapy component of community rehabilitation for clients on the Outreach Team caseload (based in North and East London and Essex) with challenging and complex acquired brain injury, within an inter-disciplinary, goal-orientated framework.
* Be responsible for and provide the specialist assessment and intervention to a designated caseload of clients with communication impairments within community settings.
* Act as key-worker and advocate for a number of the caseload; carrying out case management duties.
* Collective supervision of the Outreach Team Rehabilitation Assistant.
* Supervision of SLT students as required.
* Work within challenging socio-economic environments with patients and their families who have highly complex social problems and those from culturally diverse backgrounds (some of whom are unable to speak English).
* Actively contributing to the development of the SLT service within the RNRU Outreach Team, including initiating developments and taking a lead on projects as required in line with Trust objectives, local policy, client needs and national guidance.

CLINICAL DUTIES:
1. Develop and provide specialist assessment, diagnosis, treatment planning and intervention to patients with acquired disorders of communication and / or swallowing with complex physical, cognitive and challenging behaviour. To include utilising a high level of clinical reasoning to formulate hypotheses and inform patient focussed goal setting and treatment planning as well as utilising individual group and interdisciplinary working across a range of settings.
2. Take a key-worker role for specific patients in order to ensure continuity of care and enable a point of contact for patient/carer concerns / feedback, smooth transition through admission and discharge procedures and liaison with care managers and further services to ensure safe, effective and timely discharges.
3. Act as an expert resource in advocating for patients with complex communication impairment to enable them to understand, contribute and make informed consent to treatment options for all disciplines.
4. Discuss with patients the implications of SLT diagnoses and provide support and advice to families and carers.
5. Assess, diagnose and plan treatment programmes to be delivered by other members of the team, family, carers , and students as appropriate to their skills and experience.
6. Take a central role in managing clients with complex swallowing difficulties including identifying, evaluating risk and making referrals appropriate to their need and available community services.
