Mediplacements are looking to recruit a band 7 Clinical Pharmacist to work with a client of ours based in Essex.
The pay for this position is £27-£32 per hour, and we are looking for someone who can start as soon as possible for an ongoing duration.
Successful candidates must hold an MPharm degree, be GPHC registered and have experience working as a clinical trials pharmacist.
ROLE SUMMARY
To provide specialist clinical pharmacy in a specific area of clinical and/or operational practice, to support the medicines-related care of patients, support the provision and development of medicines management services and to participate in training, education and research as required.
* To actively contribute to, and participate in, the provision and development of pharmaceutical services, and safe and effective use of medicines, within the West Essex Partnership University NHS Foundation Trust, as part of the Trust's pharmacy and medicines management team.
* To participate as an active member of specific ward / unit / clinical teams, and support the safe and effective use of medicines.
* To contribute to the organisation and management of the clinical pharmacy service to specific wards / units / clinical teams.
* To provide clinical pharmacy and medicines management advice in order to maximise benefit and minimise risk in relation to medicines
* To support and participate in clinical audit.
KEY RESPONSIBILITIES
Clinical Practice
* Ensure safe and appropriate use of medicines in order to maximise benefits and minimise risk to patients. This will include regular review of prescriptions, identification of drug interactions and adverse effects, advice on clinical monitoring, compliance with legislation (for example, Medicines Act, Mental Health Act, Misuse of Drugs regulations), compliance with Trust policies and guidelines.
* Provide a clinical pharmacy service to assigned wards and to the multidisciplinary team caring for patients. This includes:
* review prescriptions for accuracy and legality and liaise with prescribers about actual or potential problems
* medicines reconciliation
* liaise with the Trust's pharmacy when required, to facilitate safe, secure and effective medicine supply
* facilitate the effective outcome of treatment with medicines
* monitor patients for potential and actual adverse effects of their medicines
* take steps to ensure that patients understand the purpose of the medicines and deal with any practical issues that may prevent the optimal use of their medicines
* support the planning of patient discharge with respect to medication
* demonstrate professional accountability to patients
