Mediplacements are currently looking to recruit an experienced Oncology Pharmacist for a locum role in Surrey.
The pay for this position is £27-£45 per hour, and we are looking for someone who can start as soon as possible for an ongoing duration.
Job Summary
To co-ordinate the provision of chemotherapy and other specialist drugs for haemato-oncology patients including day case patients and private patients.
To provide specialist pharmaceutical advice to medical and nursing staff on drugs for haemato-oncology patients.
To manage the Trust's e-prescribing system for chemotherapy
Key Responsibilities/Outcomes:
Clinical:
1. To co-ordinate the provision of chemotherapy for haemato-oncology patients.
2. To clinically screen chemotherapy prescriptions and to ensure that the prescription is correct and that the regime is safe and appropriate for the patient.
3. To develop systems to ensure that all chemotherapy prescriptions are screened appropriately.
4. To provide specialist pharmaceutical advice to medical and nursing staff working in haemato-oncology.
5. To provide medication counselling for patients undergoing chemotherapy.
6. To attain and maintain accreditation for authorisation to supply intrathecal chemotherapy and other forms of chemotherapy.
7. To provide specialist input into processes involving PbR excluded drugs and Cancer Drug Fund Drugs and liaise with the Principal Pharmacist Medicines Management.
8. To provide clinical advice and support for the use of immunoglobulins for haemato-oncology.
9. To liaise with the cancer clinical trial investigators and the Principal Pharmacist Informatics and Non -Medical Prescribing in the provision of clinical trials involving haemato-oncology drugs.
10. To promote the safe, economic and effective use of medicines.
11. To participate in the delivery of ward and dispensary based pharmaceutical services and maintain clinical and technical skills including:
* To implement a good framework of pharmaceutical care through effective drug history taking and documentation, review of patient medication, evaluating efficacy and toxicity of medication used, observing and reporting risk issues relating to the use of medications, when necessary.
* To undertake effective monitoring for the use of medication, adjusting dosages and route of administration through timely intervention and communication with medical, nursing and other healthcare professionals, when necessary.
* To liaise with carers, senior pharmacists, nursing or medical staff to optimise medicines management for patients, allowing best outcomes for medical interventions.
* To participate in weekend and Bank Holiday duties as required in the provision of cover for 'out of hours' Pharmacy services.
* To work as a pharmacist, providing a dispensary service including accurately dispensing, checking medication and counselling patients regarding their medication as required.
* To provide information and advice on the preparation and administration of intravenous drugs.
* To demonstrate and communicate effective and expert clinical pharmacy knowledge and advice to healthcare professionals and patients.
* Practice in accordance with the Code of Ethics of the General Pharmaceutical Council (GPhC), professional standards of the GPhC and Royal Pharmaceutical Society of Great Britain / and within the Medicines Act (1968) and subsequent legislation.
12. To contribute to and adhere to clinical pharmacy service standards.
13. To liaise with consultants in the development and updating of prescribing policies and procedures.
14. To ensure that prescribing is in line with formulary guidelines and prescribing policies.
15. To liaise as appropriate with other healthcare professionals within Primary Care to improve communication across the interface to ensure continuity of care.
16. To demonstrate an advanced level of clinical reasoning and judgement.
